The obesity market is moving from supply-constrained injections toward broader payer coverage, direct-pay channels, oral products and combination regimens.

  • Scale: Q2 tirzepatide sales were $14.87bn, creating funding and commercial reach that smaller competitors cannot match.
  • Operating leverage: 2026 revenue guidance is $85-87bn and non-GAAP performance margin guidance is 49.0-50.5%.
  • Lifecycle depth: Foundayo adds oral convenience; retatrutide adds triple-agonist efficacy; EloraTZP adds amylin biology.
  • Global runway: ex-US Q2 revenue grew 80%, despite a 36% realized-price decline, largely due to China reimbursement.

Why price discipline still matters

At roughly 24.0x our 2027 consensus check, Lilly needs both high EPS growth and a premium terminal multiple.

  • A miss in one molecule can hit revenue growth, margin and the multiple together.
  • US government, payer and cash-pay agreements exchange lower net price for volume.
  • CagriSema, oral Wegovy and future Roche or Amgen products limit assumptions of permanent category dominance.
  • Lilly had about $45.2bn of net debt at Q2 while funding manufacturing, acquisitions and buybacks.

Industry drivers

Access and price

The US GLP-1 Bridge program began 1 July. Government agreements contemplate $245 monthly injectable pricing for Medicare and average cash pricing near $346 through TrumpRx. Lower patient cost can expand treated lives, but net revenue per prescription falls.

Oral adoption

Novo said Wegovy pill exceeded five million prescriptions since launch and 265,000 weekly prescriptions by 17 July. Its Q2 sales were DKK 3.22bn. Lilly's newer Foundayo generated $98m in Q2.

Next-generation efficacy

CagriSema, retatrutide and amylin combinations push efficacy beyond first-generation GLP-1s. Commercial outcomes will also depend on tolerability, persistence, manufacturing and reimbursement.

Lilly can remain an excellent company while the shares disappoint. Tirzepatide is almost two-thirds of revenue, net price is already falling, and the base valuation assumes $47.5 of 2027 normalized EPS and a 28x exit multiple.
